Modafinil is used as a prescription smart pill to treat narcolepsy but it is also being widely used off-label as a cognitive enhancement smart pill. This is because it is one of the first smart pills to be shown to improve cognitive function in healthy people rather than sleep-deprived ones.
It does this by affecting the dopamine system which makes you more interested in things and motivated, the norepinephrine system which keeps you awake, and histamine which helps with memory and focus.
Increased Focus
Scientists have recently found that Modalert 200, a smart pill prescribed to treat sleep disorders like narcolepsy, can also sharpen our focus and boost our ability to learn. In a study published in the journal Nature, researchers showed that modafinil increases something called “fluid intelligence,” which is our capacity to solve problems creatively and think abstractly.
In a double-blind, placebo-controlled experiment, 60 healthy student volunteers were given 200 mg of modafinil or a placebo and then performed several cognitive tests. The 200-mg group improved their performance on the 5-choice continuous performance task (5C-CPT) and Wisconsin card sorting task (WCST) when compared to the placebo. However, they did not improve on a simple color naming task or self-ratings of sleepiness or tiredness before the test session.
Rare cases of multi-organ hypersensitivity reactions including Stevens-Johnson syndrome (SJS) and Toxic epidermal necrolysis (TEN) have been reported in patients receiving modafinil. These reactions may be life-threatening. Administer PROVIGIL with caution in patients with a history of these reactions.
Enhanced Memory

Widespread usage of this smart pill could undermine these conditions and make users worse off. It also risks lowering the quality of people’s lives, not just by masking poor sleep, but by creating a distinctive form of agency possible only in free time. This is an ethical concern that needs to be addressed.

The promise of Modafinil is a reminder of the power of science and technology. Moreover, it could be a catalyst for a variety of cognitive enhancement technologies including neurofeedback devices and mobile applications that monitor brain waves, as well as techniques such as transcranial direct current stimulation.
